I need a graph with a dynamic changing x-axis (last 12 months) with dates in the format “MM-YYYY”.

I am using a date hierarchy to get the dynamic part working. But I can’t figure out how I can get the right format. Currently it is either a full date (dd-MM-YYYY) or just the month (MM). I heard that a form could help here, but I can’t get this conversion done and even if I would… How can I use this as a header/axis?
