In general, if there are any display folders set, then they will be used to arrange the measures.
Any measure that is not under a folder is placed outside (like in your image).

The one exception is that if you don't have any folders at all, then the measures a arranged by the measure groups.

 

There is no mixture of measure groups and folders together.  If you want to use folders and sub-folders you need to place each measure in the folder you'd like.

To create nested folders use "\". For example the folder "root\sub" will create 2 folders so that "sub" is nested under "root". You can then place a different measure in folder path "root" and get the nesting of measures as you wanted.
